IMPORTANT NOTICE

Effective with your August, 2009 statement, Delta Dental of Tennessee will no longer include subscribers' full Social Security numbers on groups' monthly billing statements. Subscribers will instead be identified by the last four digits of their Social Security number. Delta Dental is enacting this policy to protect subscribers from the possibility of identity theft.
