At Delta Dental of Tennessee we strive to provide superior Customer Service. It is important to us to answer every inquiry quickly, professionally, and accurately.

Currently, we only offer employer-sponsored group coverage. Please feel free to have your employer contact our Marketing Department for more information.
For the most recent program type information, please contact our Customer Service department @ 1-800-223-3104.
Information for dependents 17 and older must be obtained via phone.
You may print an ID card using our Consumer Toolkit or you may request an ID card via phone by calling 1-800-223-3104 and pressing 4.
Some of our plans cover two cleanings at 100% of the Maximum Plan Allowance per 12 consecutive month period (not necessarily a calendar year). Certain restrictions may apply, please speak with a DDTN Customer Service representative for more information.
DDTN has an extensive list of participating providers which can help to reduce your out-of-pocket expenses. However if you choose to see a dentist who is not a provider with Delta Dental, you may be still be eligible for partial reimbursement. PRIOR to your visit, we strongly recommend that you submit a pre-treatment estimate or contact a DDTN Customer Service Representative for more information.
Claim forms are available online. Either you or your dentist may complete the form and attach a copy of your bill. Completed forms may be faxed to 615-244-8108 or submitted via mail to: Delta Dental of Tennessee, 240 Venture Circle Nashville, TN 37228.
