Broker Forms

	EFT Form Did you know that Delta Dental offers direct deposit? Direct Deposit is also known as Electronic Funds Transfer (EFT). It is the transfer of funds from Delta Dental's account to your checking or savings account.
	PHI Release In order for Delta Dental of Tennessee to release Protected Health Information to you for one of your clients, you must have your client complete and sign this form.
	Agent Fee Agreement for Individual Plan This Agent Fee Agreement must be completed if you enroll members in the Delta Dental Individual Plan. One agreement will cover all individuals enrolled.
	Agent Fee Agreement - Business Associate Addendum When an agent/agency writes its first group with Delta Dental of Tennessee, the Agent Fee Agreement Business Associate Addendum must be signed. If commission is paid to the agency, the form can be completed by the agency with Tax ID number used. If commission is to be paid to an individual agent, the agent must sign the form using his/her social security number.
	Agent Fee Agreement 2009 The Agent Fee Agreement needs to be completed and turned in with each new group you submit. On this form there is a commission schedule for standard commission levels.
