	EFT Form Did you know that Delta Dental offers direct deposit? Direct Deposit is also known as Electronic Funds Transfer (EFT). It is the transfer of funds from Delta Dental's account to your checking or savings account.
	Agent Fee Agreement The Agent Fee Agreement needs to be completed and turned in with each new group you submit. On this form there is a commission schedule for standard commission levels.
	PHI Release In order for Delta Dental of Tennessee to release Protected Health Information to you for one of your clients, you must have your client complete and sign this form.
	Agent Fee Agreement for Individual Plan This Agent Fee Agreement must be completed if you enroll members in the Delta Dental Individual Plan. One agreement will cover all individuals enrolled.
