How will a predetermination help me?
A predetermination helps you and your dentist understand what treatments are covered under your dental plan and estimates what your out-of-pocket expense will be.
When should I request a predetermination from my dentist?
- If your dentist is recommending extensive treatment
- If you need an estimation of your out-of-pocket expense
- To determine benefit coverage and plan limitations
How long does it normally take to receive a predetermination?
If all information is received upon the initial submission of the predetermination, please allow 10 – 14 business days for processing
Please remember, a predetermination is only an estimation of your benefits. A predetermination does not guarantee payment of benefits and should only be used as a tool for you and your dentist to plan your treatment.
